Nomination Forms

 

For the Gold Star and Blue Star nomination forms you can use Interactive PDFs (top two choices in the column to the right) which allow you fill in the forms right on your computer.

 

Gold Star: Upon completion of the Gold Star form, hitting the SUBMIT button will automatically generate an e-mail addressed to the executive director of the Calvinist Cadet Corps with the form as an attachment.

 

Blue Star: Upon completion of the Blue Star form, hitting the PRINT button will open the dialog box for your printer so you can print the form.
